Integrated Care Partnership PCN joining DHC GP Federation

We are delighted to announce that Integrated Care Partnership (ICP) PCN will join DHC GP Federation effective from 1 April 2022.

ICP will be joining the PCNs of Dorking, Redhill Phoenix and South Tandridge. Our new collaboration brings together like-minded GPs with a shared vision of PCN development that will strengthen our Federation.

In these challenging times, the benefits of working together to deliver innovative services at scale has been brought into even sharper focus. ICP has a long history of developing patient services and share our vision to bring a broad range of quality healthcare closer to patients’ homes and to carry out transformational work that improves patient care in the longer term. As a GP Federation in Surrey Heartlands, we look forward to building on our role within Surrey Downs Place and East Surrey Place. Together, we will work to integrate primary care with the wider health and care community.

Integrated Care Partnership (ICP) is the largest General Practice in the Epsom and Ewell area with a list of circa 33,500 registered patients. It has sites based in and around Epsom and Ewell: Old Cottage Hospital, Fitznells Manor Surgery, Stoneleigh Medical Centre and Cox Lane Surgery. All four sites work together with our dedicated team of doctors, nurses, paramedics, pharmacists, physios, reception and administration teams to ensure that our patients are provided with the best possible holistic medical care.
