Senior Clinical Pharmacist
Band 7 or 8A – ideally looking for prescribing pharmacists
Our successful and forward thinking organisation is looking to recruit a part-time Pharmacist to work within our four Dorking practices (Leith Hill, Medwyn, Dorking Medical/Westcott and Brockwood Medical Practices).
DHC is a dynamic GP Federation providing NHS services throughout Surrey. These include out-patients, psychological therapies, diagnostics and minor surgery. We are a constantly evolving organisation with a real ethos for providing quality and local services to patients. A ‘can do’ attitude is a vital asset at DHC and we are looking for like-minded individuals to work with us who share this same approach and work ethic.
The successful applicant supports and works alongside other pharmacists in the four Dorking GP practices. The pharmacists will be a key part of a multi-disciplinary team in a patient-facing role and work within practices to undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older patients, residential care home patients and those with multiple co-morbidities.
If you are successful, you will provide primary support to general practice staff with regards to prescription and medication queries. You will help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices.
We will need you to provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
We are looking for a knowledgeable and skilled professional with great customer service skills and the ability to develop themselves in our growing organisation. We require the successful applicant to be registered with the General Pharmaceutical Council. In addition, it would be desirable if you are a member of or working towards Faculty membership of the Royal Pharmaceutical Society, have a Masters degree in pharmacy (MPharm) (or equivalent), have specialist knowledge acquired through postgraduate diploma level or equivalent training/experience. Finally, it would also be desirable if you are an independent prescriber or working towards/intent of gaining independent prescribing qualification.
